Who is eligible for StarRider? |

i) Driver - anyone who resides in Malaysia with a valid driver’s license under the regulations of the Malaysia Road Transport Department
ii) Passenger – anyone in between the ages of 3 to 70. |

Am I eligible for this plan if I am already disabled? |
| A2. |
Yes, although we cannot change the policy to suit your specific needs, such as removing cover for loss of sight for someone who is already blind, you may still take this policy and benefit from all aspects of the cover except the existing disablement. |

I already have an existing PA with your company. Can I convert this to StarRider? |
| A3. |
Each PA offered by our company has different features, which correspond to the premiums charged. Therefore you cannot convert your existing PA. You may enrol in StarRider or alternatively either choose to continue your existing PA until expiry or terminate and opt to receive a refund (short period rate premium). |

Can I pay the premium by instalments? |

No, because this is an annual premium payment plan with premium as low as RM60 only. |

Will the premium increase over time? |
| A2. |
No, unless you top-up your sum insured or else you will pay the same premium rate (calculated at the point of sign-up) throughout the term of the policy. |

Can I sign up for Auto Renewal with my credit card? |

Yes, you can enrol for this plan with Auto Renewal Instruction where we will automatically charge to your Credit Card (visa or master) for renewal of yearly premium. |

How do I enrol for StarRider? |
| A4. |
Just fill-up the proposal form and submit it together with the premium payment through our appointed Agents or directly to our Branches office nationwide.
You may choose any of the following payment options:
• Pay Cash/ Cheque/ Credit Cards at UAGIB Branches or to Agents;
• Fax the proposal form to UAGIB Call Centre with details of your Credit Card;
• Call Toll Free 1800-888-990. |

Do I need to pay stamp duty? |

Yes, a stamp duty of RM10 is payable for each new or renewal policy and it shall be included in the first payment. |

If I have a personal accident policy with another company, can I claim accidental damages under both policies? |
| A1. |
At Uni.Asia General, we will pay out regardless of how many other personal accident policies you hold as long as you have declared the insurance coverage at the time of taking out the policy with us or afterwards. However, we suggest you also check with your other insurer as their terms and conditions may vary. |

How do I make a claim? |

In the event of a claim situation arising, notify UAGIB immediately at the following numbers:

• Call any of the UAGIB branches;

Is there a deadline to file a claim? |

Yes, there are deadlines of how long after an accident you should file a claim. The claim for injury causing the death and permanent dismemberment must take place within one hundred eighty (180) days while claim for medical expenses incurred must be within fifty two (52) weeks after the occurrence of accident. |

What are the documents my family or I need to complete and submit? |

The claimant will need to complete and submit the Claim Form to UAGIB as soon as possible together with the following documents:
• For Non-fatal case:
i) Medical report
ii) Original medical bills & receipts
• For Fatal case:
i) Copy of police report
ii) Copy of death certificate
iii) Copy of post-mortem report (if available)
iv) Burial certificate
v) Letters of Administration

Do I need to nominate any beneficiary? |

If you have purchased this PA policy, you are encouraged to nominate one or more than one beneficiary and ensure that they are aware of this PA policy. In the event of your disablement or demise, your nominated beneficiary will be allowed to claim and receive the compensation as planned. (Note: Nomination form does not apply for MUSLIMS) |

Can you tell me why it is necessary to have the witness's signature on the nomination form? Can I ignore it or have my family members to sign it?

Yes, it is important to have the witness' signature to prevent future disputes or forgery. Any of your family members can be the witness. |

What will happen if death occurrence without nominating any beneficiary? |
| A7. |
In the event of your demise without nominate any beneficiary; we will pay the proceeds of any claims into your estate for distribution according to your will, so that the executor or administrator of your estate could make arrangement as per what you laid down in your will. If there is no will, we will pay according to the intestacy rules of Malaysia Distribution Act 1958 (amended act 1997), which is usually to your next of kin and/or any dependants. |

What is “Short Period Rate”? |

“Short Period Rate” means the Insured may cancel the policy at any time after receipt of the policy. The policy will be cancelled and the earned premium shall be computed in accordance with the below short rate table used by the company at any time of cancellation.
• Not more than one month : 25%
• More than 1 month but within 3 months : 50%
• More than 3 months but within 6 months : 75%
• More than 6 months but within 9 months : 90%
• More than 9 months : 100%

Yes, this plan covers double indemnity in the event of accidental death that occurs during a nationwide public holiday. |

Does this plan cover death or TPD arising from Tsunami, natural disasters and/or terrorism? |

No, this plan does not cover Tsunamis, Earthquakes, Natural Catastrophes and/or Terrorism. |

Are there any exclusions? |
| A4. |
Yes, standard PA exclusions apply. Generally, the benefits shall not be paid if the Insured Person sustains bodily injury due to exclusions below:
• If the driver does not hold a valid driver’s license to drive the vehicle or if not qualified for holding or obtaining such a valid driver’s license under the regulation of the Malaysia Road Transport Department;
• While the vehicle is used for illegal business as an unlicensed common carrier;
• If the vehicle is used for hire or reward, road rally pace making, speed-testing or use for any purpose in connection with the motor trade;
• War or contamination by war, invasion, act of foreign enemy, hostilities or warlike operations, mutiny, civil war, rebellion, revolution insurrection, conspiracy, military uprising, martial law or state of siege;
• Bacterial infections, suicide or self-inflicted injury, pre-existing physical or mental defect or infirmity, HIV and AIDS related diseases;
• Childbirth, miscarriage, pregnancy or any other complications thereof;
• Criminal acts or terrorism;
• Influence of intoxicating liquor, alcohol or drugs.
